Medical Credentialing Program

Program Description

The Office of the Clinical Director serves as the liaison for credentialing services at NIDCR. Our dedicated Credentialing Coordinator is here to assist you throughout the credentialing process for medical staff membership and clinical privileges at the NIH Clinical Center.

While applications are formally submitted through the Office of Credentialing Services, our team is committed to providing guidance and support every step of the way. We will help you navigate the required forms and documents, ensuring a smooth and efficient process.

If you are providing direct patient care at the NIH Clinical Center, you will need to apply for medical staff membership and clinical privileges. 

Eligibility
Eligibility

Doctors of medicine, osteopathy, dentistry, optometry, and podiatry who are qualified by education, training, experience, and evidence of clinical competence are eligible to apply.

How to Apply
How to Apply

Submit an Application for Initial Appointment to the Medical Staff (NIH-2688), a Record of Participation in Patient Care (NIH-101-1), and the appropriate form(s) for Delineation of Privileges (NIH-2677) through the Office of Credentialing Services.

Required Documents:

  • Letter of recommendation from the sponsoring Institute or Center (IC) branch chief, Clinical Center department head, or Clinical Director
  • Written verification of all positions involving patient care over the past 10 years
  • Two letters of reference from peers or supervisors
  • Copy of certificate of completion for each post-graduate training program
  • Curriculum vitae and bibliography covering all medical education and positions held in the last 10 years
  • Original or notarized copy of the medical school diploma or official medical school transcript
  • Copy of specialty or subspecialty board certificate(s)
  • Evidence of current cardiopulmonary resuscitation (CPR) certification
  • Evidence of current, unrestricted licensure
  • Educational Commission for Foreign Medical Graduates (ECFMG) certification (if foreign medical graduate)
  • Valid Special Volunteer Agreement (form NIH-590-2) for special volunteer appointments
  • Copy of U.S. Permanent Resident Card (USCIS Form I-551), if applicable

Important Notes

  • Short-term appointments are for one year and are not eligible for renewal or reappointment.
  • The CEO may grant a licensure requirement waiver for Civil Service applicants only.
  • Some foreign citizens may be eligible for a license exemption through the NIH Visiting Program.
